Membership begins as low as $30 per month and incorporates cleanings, exams, fluoride, x-rays, and discounted dental services. When you join, you will receive a $500 credit towards orthodontic treatment at Popat Orthodontics for our all registered family members on this plan.

What is MPD’s Pink Bow Tie Membership?

Madison Pediatric Dentistry (MPD) Pink Bow Tie Membership is a subscription plan where the patient or member in question directly pays the dental specialists for the services delineated in the Membership Plan.

  • Complete set of Radiographs
  • Two Periodic Exams (6 months apart)
  • Two Prophylactic Dental Cleanings
  • Two Oral Cancer Screenings
  • Two Fluoride Treatments

TeleDentistry Services:

  • Text messaging for follow up care
  • Texting pictures related to your child(s) oral health between visits
  • Email Messaging & Appointment scheduling

Promotional Services:

  • $500 Orthodontic Credit (Must be currently enrolled in MPD’s Pink Bow Tie Membership to use the credit.)

1. What is excluded from my Membership?

This plan does not include ongoing treatment with Major Surgeries, Minor Surgeries, Dental implants, Anesthesiology Fees and Medication.

2. Would I be able to utilize my Dental Insurance to pay for extra care beyond this Membership Plan?

No, the MPD’s Pink Bow Tie Membership is its own care plan. Therefore you can’t use your insurance to pay for additional services not covered by the membership.We offer a 15% discount on additional diagnostic and x-ray films, restorative dental care and nitrous oxide.

3. Why should I join MPD’s Pink Bow Tie Membership?

We developed this affordable plan for your children to receive quality care at our private practice so you don’t have to navigate through insurance companies and your HR department. This plan clearly delineates your child(s) dental benefits and you will know all potential fees in advance. There are no hidden fees or financial surprises. We are additionally offering an Orthodontic Credit as a thank you for joining.

4. How much does the Membership cost?

Membership starts at $30 dollars every month, per child for children ages 0-14. Children ages 14 and up plans are $40 every month, per child. A one-time Dental Assessment and Enrollment Fee of $99 is due at the time of registering each child in this plan. If you drop your Membership and decide to re-select sometime in the future, Dental Assessment and Enrollment Fees will be expected. Your first month’s charges and Dental Assessment Fees are expected when you enroll.

5. Would I be able to utilize my Health Savings Account (HSA) to pay for my Membership?

No, you can not utilize HSA to pay for Membership.

6. Can my membership be canceled?

We reserve the right to cancel your membership due to non-payment, expired credit cards, back payment owned to MPD or other results resulting in membership cancelation or termination. Your Orthodontic Credit will be voided at the hour of cancellation.

6. What is the Orthodontic Credit, and how would I use it?

The Orthodontic Credit is a courtesy gift for joining in MPD Pink Bow Tie Membership. To utilize your orthodontic credit, you must currently be enrolled in our Pink Bow Tie Membership and have no outstanding balance at our office. The Orthodontic Credit is only valid at Popat Orthodontics and may only be used once per member. Your Orthodontic Credit is dated as the same date of your enrollment. We reserve the right to change, alter, cancel, terminate or void this credit as necessary. The Orthodontic Credit is not redeemable for cash, or for any other purposes other than a credit towards orthodontic care at Popat Orthodontics.

7. Do I get one Orthodontic Credit for each part enlisted or per Membership?

You get one Orthodontic Credit for each member enrolled in our MPD Pink Bow Tie Membership Plan. Each child enrolled in our MPD Pink Bow Tie Membership plan will receive one Orthodontic Credit. Only one credit may be used per child. We reserve the right to change, alter, cancel, terminate or void this credit as necessary. The Orthodontic Credit is not redeemable for cash, or for any other purposes other than a credit towards orthodontic care at Popat Orthodontics.

8. What if I decide to cancel my Membership?

If you decide to cancel, please notify our office immediately of your intent to do so. Cancellation forms can be emailed to you and must be signed and submitted before the next scheduled payment. There are no refunds if you cancel after your transaction has occurred. Your service will continue for the duration of the month of cancellation. For example: If you cancel on the 3rd of the month, and have paid, your enrollment will continue through that month and terminate on the last day of that month. Additionally, if you have received a promotional Orthodontic Credit, and are not currently a member, your Orthodontic Credit is void.

9. What if I would like to re-enroll in MPD Pink Bow Tie Membership?

To re-enroll in the Membership, you will need to fill out all forms pertaining to MPD Pink Bow Tie Membership. Additionally, you will need to pay the Dental Assessment and Enrollment Fees of $99 per child. Your Dental Assessment and Enrollment Fees and 1st month payment is due at the time of re-enrollment.

It is Madison Pediatric Dentistry responsibility to inform you that, the MPD Pink Bow Tie Membership is not insurance, “The organization (Madison Pediatric Dentistry) facilitating the Membership is not an insurance company and the Membership guidelines and plan operation are not an insurance policy. Participation in the membership or a subscription to any of its documents should not be considered to be insurance. Regardless of whether you receive treatment for medical issues through the Membership, you are always personally responsible for the payment of any additional medical expenses you may incur.”
